Immunization Records:

 

CDO is required to have a copy of your child's current immunization record on file at all times. This must be on file by the first day of attendance and CDO must be notified of any updates of shots throughout the year.

 

Illness:

1. CDO will accept only children free of illnesses. We depend on parents to help us maintain this policy. If your child is under a physician’s care, they will advise you when the child may return to school.
2. CDO must be informed if your child has a communicable disease, so that we can inform other parents.
3. We cannot keep children who are running a fever.
4. Parents will be notified to pickup their children, if they appear ill or are running a fever.

 

Medication:

1. If a child requires medication during CDO hours, the parent must sign and date a medical release form giving the Director permission to administer medication.
2. Medication must be in the original labeled container and must be kept in the Director's office... away from the children.
3. The Director will document all doses of medication administered to the child.
